Choose who will make health care decisions for you if you are unable to make them yourself.

What would happen if you became incapacitated tomorrow? Who would take care of you? Who do you trust to make decisions for you about health care, medical intervention, and personal care?

The traditional living will only allows you to make statements about your end-of-life choices, but with this kit you can draw up legally binding directives that appoint a substitute decision maker and give instructions for how decisions are to be made.

This kit contains information and instructions for all provinces and territories. It includes directives and forms you can fill out to leave clear, precise instructions that express your fundamental values and beliefs.
